Injured at Work?

If you are injured while on the job, you need to immediately do the following:

  • Report the incident to your supervisor and/or building contact (for a list of contacts, click here)
  • Fill out a FIRST-REPORT-OF-EMPLOYEE-INJURY-or-ILLNESS-fillable-form, no matter how significant or insignificant you may believe the injury to be. Your supervisor and/or building contact will provide you with a copy of this form and can assist you in its completion.
  • ALL MSDPT employees who sustain an injury requiring basic medical attention need to see a physician at one of the following locations. Please do not go to any other physician as your personal health insurance may not cover an injury sustained at work, and worker’s compensation may not cover initial medical expenses incurred at a location other than those listed below.

Franciscan Working Well (formerly St. Francis)

This facility should be used for daytime needs.

This facility should be used for night-time needs.

Hours of operation: 7 pm-11 pm

If an injury is serious or life threatening, an ambulance should be called or transportation provided to the closest emergency room. However, less threatening incidents need to follow the above guidelines.

Failure to use the approved clinics listed may nullify your Worker’s Compensation coverage.
